Issues fixed

Jittery mouselook when mouse polling rate exceeds 125 Hz

Wrapping the old DirectInput calls into DirectInput 8 seems to fix the issue[4]
  1. Download DirectInput wrapper.
  2. Extract it to <path-to-game>.
  3. Add the line InputRate=60 in <path-to-game>\autoexec.cfg.
  4. Decrease mouse sensitivity in-game and increase it on the mouse driver control panel's DPI settings.

Starts on second monitor

Set display manually[5]
  1. Go to the installation folder and edit autoexec.cfg.
  2. Locate the line "carddesc" "\\.\DISPLAY2".
  3. Replace it with "carddesc" "\\.\DISPLAY1".

Notes

If the line does not already exist, add it manually.

Game has a 2048 pixel resolution limit

Use dgVoodoo 2[6]

According to System Informer, the game loads d3dim700.dll. dgVoodoo2's traces point to Direct3D 7 (e.g: "Direct3DDevice7").
