Graphics are corrupted in DOSBox when the Video installation option is set to VGA.[1]
Set Video to PS/2 VGA[2]
  1. Run <path-to-game>\HINSTALL.BAT.
  2. Set Video to PS/2 VGA.
  3. Press ↵ Enter twice to save the change. The game will now display correctly.
