I don't see the difference. Screenshots taken with GameBar(Win+Alt+PrtScr) have max luminance of 287 nits(via WCG Image Viewer) whether that variable is true or false. I chose a scene where sun is directly visible as a brightest object.

Using SpecialK I enabled HDR and made it to be around 1000 nits, my HDR TV's max value.

P.S. I've reverted your edit. HDR being false automatically generates note with links to UE4's native HDR method and Special K retrofit. HDR true should only be used for games with native support, when game has such setting in a menu or automatically enables it.
