There are four crystals which are to be positioned on the underwater stone alter. The crystals are three-sided and have a symbol on each of their three sides. The symbols on the first and second crystals are proper, but the third crystal has a spear symbol where it should have a fish symbol and the fourth crystal has a fish symbol where it should have a spear symbol. This is not a game-breaking bug because the puzzle can still be solved through trial and error (assuming the player eventually resorts to trial and error).
If the clothesline is combined with the clamp before the inflated rubber duck is combined with the clamp, a bug is triggered which will make it impossible to obtain the key on the subway tracks.
